西地那非是cGMP特异性5型磷酸二酯酶（PDE5）的一种有效选择性抑制剂。西地那非可用于治疗勃起功能障碍和肺动脉高压。NO可激活鸟苷酸环化酶，从而导致cGMP水平增加并产生平滑肌松弛。西地那非可通过抑制负责cGMP降解的PDE5来增强NO的作用。
